Volunteers in Case Management?

    I'm restructuring one of my direct report areas (1.5 paid employees and 6 volunteers) which has been connecting self-pays with Rx and PCP help. Now that Medicaid expansion is coming to North Carolina, we're dropping the Rx contract. Do any of you have volunteers that help with Case Management functions? Part of the conversation also includes the Health Inequity NPSG, as the area's past role has worked to share community resources with those that need them.

    We do. We're a long-term rehab hospital so I have a weekly volunteer who visits those close to discharge with a summary sheet showing any adaptive equipment coming (i.e. wheelchair) and the name and contact info of the vendor. She also makes follow up calls about 2 weeks post-discharge to ensure everything arrived, and takes down questions to pass on to the CMs if there are any from the patient. It's freed up a lot of time for our CMs and the volunteer really enjoys the work.

    I was just in a meeting about a similar situation yesterday. Thanks for sharing about that role, Alex! Always helpful to hear what other places are doing with their folks. :)
