    Hi Mary,

    - We require (and pay for) background checks for anyone over 18, and drug screens for all.

    - Re the vaccines listed, we require all of those EXCEPT HepB. Fully vaxxed for us is two doses of anything except J&J and at least two weeks post the second dose.

    - I haven't had anyone ask for exemption for COVID (and I'd say no because we have lots of patients with high acuity and on vents) but we'll give religious/medical exemptions for flu only.

    - TB testing is only when you first start and has to be within the past 90 days of you presenting the results. (So if you bring them in today, it can't be older than about March 28.) We'll also accept a chest xray for those who get false positives; that's good for 5 years. 

    - We do not require a physical. We used to ask for a health history but discontinued that several years ago.

    - We also require all volunteers to complete and pass compliance training similar to what staff has to take annually.

                           Flu vaccine annually                  Yes  - no exemptions 

                            COVID vaccine                        Yes - we do accept medical/religious exemptions -  They must have 2 at this time, but I am taking copies of boosters.  

                 TB testing  Yes  - once and then questionnaire filled out each year.  

                Physical                                                                        No

    If you require any of the above does your facility pay for the testing or vaccines?     WHC pays for criminal background check, Flu, and TB test.

    Any other comments or notes about onboarding: I do an in person interview, 1 reference (great recruitment tool).  All volunteers must attend orientation, part self study/part in person, then specific orientation.

    Does your facility offer exemptions for any of the vaccines or immunities? Yes. For Covid & flu vaccines, there is the ability to request exemption for religious or medical reasons.

    TB testing - Yes; once

    Physical - No

    If you require any of the above does your facility pay for the testing or vaccines? Yes, we provide TB testing for adult volunteers. I require college students to provide their own. We pay for the background check for all. Our facility, thru Employee Health, provides flu shots each year and we provided the covid vaccine in the beginning.

    Any other comments or notes about onboarding: In addition to the health-screening and background checks, we require HIPAA education, Code of Conduct education, patient experience training, and safety orientation.

    Flu vaccine annually - Yes, can sign declination if they don't receive - departments have the right to require volunteers in their areas to be vaccinated

    COVID vaccine - Yes

    If Yes what is the definition of fully vaccinated? 2 weeks or more since completed administration of a single-dose COVID-19 vaccine or the administration of two doses of a multi-dose COVID-19 vaccine. I do document boosters, although they are not required at this time.

    Does your facility offer exemptions for any of the vaccines or immunities? Yes. For Covid & flu vaccines, there is the ability to request exemption for religious or medical reasons. 

    TB testing - Yes, during onboarding. Then annually a form detailing TB exposure risk completed. 

    Physical - No

    All above are paid for by hospital.

    If the volunteers will perform patient care, they are fit tested at no cost to them.  As part of the volunteer orientation, they now need to attend a donning and doffing session given by our Professional development department.  The volunteers also read and sign an attestation for wearing the mask and socially distance.
